Long time No Post

It has been awhile since we posted anything here – sorry! I could probably easily become one of those hardcore bloggers that post everyday, but I found that it was cutting into other things that I really needed to do. I guess you will have to settle for periodic updates!

These past 2 – 3 weeks have been a little hard because Jono has really been struggling with asthma. He was sick in bed with a fever for a few days, but he has mostly just been finding it hard to breathe. I think, as of last night, he might finally be starting to feel a little bit better.

Josiah has been doing well. He has continued to gain weight – although I have really cut back on the calories he is getting to slow down the gain. He is now a few ounces over 23 pounds. He is also continuing to develop new skills. One of his favorite things to do now is climb the stairs. The problem is that he doesn’t yet know how to get down the stairs, so we often have to rescue him!

The feeding department has been a little rough. He has been throwing up again – not nearly as often or as much as before – but it is still annoying. I have not been able to get the same amounts of solids into him as I was before. I am trying to figure out what exactly I am doing wrong. I am currently consulting with Ben to get us back on track. The good news is that Josiah continues to drink well. That is such a relief, because no matter how badly he eats the solids I know I can always get him to drink. One of these days we will get this eating thing under control!

Well, I think Josiah may be waking up – so that’s all I have time for now.

As of yesterday it was officially a month since we first went to CA and started feeding therapy. So I thought it was about time to see how Josiah has changed since then. Today he hit 22 pound mark – 3 pounds heavier than he was a month ago! Can you see the difference?

Second week home

I feel a little handicapped as I type this – “why?” You ask. Well it may have something to do with the fact that the shift key and several others are missing from my computer keyboard! Apparently Macs did not undergo testing by infants before they were put on the market. They may look sleek but definitely not sturdy. I am thinking about starting a campaign to diversify their product line to include Macs for Moms – sleek and sturdy computers!

Well it has been a full week since being back from the clinic and things continue to go well with the feeding. I haven’t been blogging as much about it since there hasn’t been that much to report. It is something I just have to remain very consistent with for awhile. I have been able to reduce the time it takes me to feed him the solids a little bit, but I still haven’t been able to increase the volumes I get into him. Hopefully that will improve by the end of this week.

Hijacked…

No, I’m not talking about the French Yacht off the coast of Somalia at the moment… I’m talking about this blog. A nice little blog which has been hijacked by feeding issues…. No I really don’t mind. I’m all for Shari writing the blog.. all for it. I know how it used to be only a few months ago – she would not get much rest the whole day which was basically feeding Josiah – so she used to visit all the feeding boards, blogs, web sites, looking for ways we could get Josiah to feed. Now if our experience can in some ways be instructive to other parents in a similar situation I love it.

We’ve been home a week now and Josiah is continuing to eat by mouth – there have been some ups and downs with the solid intake, I guess there is no magic bullet that you can just learn how to eat in one session – so Ben has rightly encouraged us to stick with the protocol and things will get better – and I think they have even in this week. He is drinking like a champ – but still doesn’t like to eat the solids that much – but I do notice a definite decrease in fussing. The best thing is he is getting it all by mouth and is gaining weight – three weeks ago he was 18 1/2 pounds – today he is 21 1/2 pounds – he has gained on average a pound of weight a week and he is crawling everywhere now.

Meanwhile getting back into the work saddle has been somewhat daunting this week, I’m trying to fight off a chest cold and also putting together the new plan for the 2008/9 Media Apprenticeship – we will have quite a number of Media Apprentices graduating soon and we need to recruit 30 more for June 1. I am also preparing all the Media Promotional Packet for TheCall DC – They gathered 400,000 8 years ago – we want a similar mobilisation this August. In addition I have to prepare to teach Church History for over 60 students 3 hours a day starting in 9 days – so I’d better get studying – I love this stuff. I went into teaching this a few years back thinking Church History was inspirational – I quickly learnt it is a very valuable subject, but more instructive than inspirational.

I’d better sign off and get on with the studying….

Changes

So I told Jono yesterday, “If I am going to keep posting onto your blog we need to change the name!” For those of you who might not have noticed before – the name of this blog used to be – “Journey of an Englishman in Kansas City”.

In the world of Josiah – things keep progressing slowly in the area of feeding. I think I may have made a breakthrough discovery last night as to why I am not getting as large amounts of solids into him as before. So this morning I made a few changes in my feeding technique and the feed seemed to go a little better than it has been for a couple of days. It will probably take a few more feeds to get back on track though. Not much else to report in this area.

Josiah has really been crawling a lot now – and fast. He loves his new freedom to explore and I am finding out all of the things that I did not make “baby safe” before we left for CA! (he seems to find them all) For some reason the rims of all of my metal trash cans seem to make great teething toys! (disgusting) Why don’t they make teething toys that babies will actually use – he doesn’t seem to like anything that was designed for teething!

The biggest obstacle we are having with him developmentally now is tight tone in his ankles. He doesn’t like to bend them when pressure is put on them. I have had several different PTs point this out to me throughout his short life. This is making it hard for him to learn how to stand or walk. It doesn’t seem like it is going to be too big of an issue though – just something he may need a little therapy with (nothing we havent done before!). So he has his old OT (occupational therapist) coming to work with him on Friday and a new PT (physical therapist) coming to work with him on Tuesday! This kid has had more therapists in 1 year than most people ever see!

For those of you who enjoy reading blogs, you may want to check out another blog that has been getting a lot of attention lately. http://cfhusband.blogspot.com/. It is a blog from a Christian couple in NC who had been trying to have a baby for 2 years with no success. Their attempts were complicated by the fact that the wife, Tricia, has cystic fibrous which usually makes a woman infertile. (Despite this fact though, Tricia had been praying for a miracle pregnancy.) Her disease had been getting worse and she needed a double lung transplant, but the day that she was supposed to be admitted to the hospital she found out she was pregnant! Many doctors told her to abort the pregnancy, since it made her ineligible to receive the receive the transplant she so badly needed – but the couple didn’t. She got so sick during the pregnancy that the doctors had to deliver the baby at 25 weeks to save their lives. It has been 11 weeks now since the birth and just today Tricia received a double lung transplant! Gwyneth, their daughter is still in the NICU, but doing really well. It is a very interesting story and blog. Tricia is actually from south NJ where I grew up!

Reflecting

I just thought I would take a post to explain some of the lifestyle changes in being able to feed by mouth rather than tube. Even when I now have a bad feed with Josiah – I would take that any day over tube feeding! A typical day with the tube looked a little like this:

6:00am – get up and try to get in a feed while he is still sleeping (there was less of a chance he would throw it up while sleeping). Get about half way through the feed when he suddenly wakes up and throws up. Change his clothes, changes your clothes, clean the carpet and whatever else he threw up on. Now he is awake – so we are as well! Start over with the feed. (this process would sometimes go on for over 2 hours – because he would keep throwing up – then we would wait maybe 20 minutes and try and get some more into him.
9 or 10 am (depending on how the last feed went) – try and feed him again. Most likely we would get about 2 1/2 to 3 ounces in when he would start acting uncomfortable. We would then stop the feed and wait for him to digest some – the whole time trying to keep him entertained so that he wouldn’t throw up. About 20 minutes later start the feed again – the whole time watching for cues that he was about to throw up so that you could stop the feed. Most likely we never got the whole amount into him or he ended up throwing up some of it. Clean up all the things he threw up on.
Anyway – this process pretty much went on all day until about 11 or 12 at night. We had to get in at least 6 feeds a day – but I think I usually ended up doing about 10 mini-feeds a day!! That is why I never really went anywhere. There was not enough time in between feeds – plus if I put him in the car too soon after a feed the car, carseat and him were usually covered in throw up! The worse part of all is that he would often throw up up to an hour or 2 after the feed was over. It pretty much felt like we were living with a lidless jar of water that could tip over at any point!!! If you held him the wrong way, spooked him, made him cry, etc, etc, etc….. he would throw up!! We were constantly trying to figure out if he had enough liquid in him to keep him hydrated.

On top of all that he would often vomit up his NG tube along with the feed. This meant that we were sometimes putting the tube in up to 5 -6 times a day! This was not fun. I also did at least 2 loads of laundry a day with all of the dirty clothes. This is not even to mention the number of times we had to clean the carpet! I cannot wait to rip that gross carpet up.

That was Then – This is Now:
We feed him 3 feeds a day plus 2 drinks (which take less than 10 minutes!) Although the 3 feeds are still taking me about an hour each (plus time to setup and clean up) it is 100 times better than before!! I am still not quite proficient in feeding Josiah yet so these times should drastically improve over time. The best part of it all though is – he is not throwing up after feeds anymore!!!!!!! He still throws up some while being fed solids, but they are not the disgusting huge projectile throw ups from before. What totally amazes me is that he almost never throws up drinking liquids now. It used to take over an hour sometimes to get 5 oz into him and he would usually throw some of it up. Now he can drink 7 oz in about 10 minutes and be fine!! Amazing!! We can also leave the house because he doesn’t throw up in the car!
Thank you Ben for teaching us how to feed Josiah! It has drastically improved our lives! Thank you to everyone who supported us in this process. I can actually come visit you now!

Readjusting

We have become rather slack in our postings since getting home – sorry! Josiah just went down for his morning nap so I decided I should write a quick update. (this is Shari, by the way) Things have been going pretty while overall. It has been a little rougher since coming home, which was to be expected, but not too bad. Josiah continues to gain weight – if he keeps gaining as quickly as he has been we will need to cut back on the calories pretty soon.

I have been having a little trouble getting the consistency of the food right. Ben had an amazing assistant named Jessica who always took care of getting the food just right – I am appreciating her skills even more now. It makes a big difference in the fed if the food is too runny or too thick! With the food not quite right I was having to be a little more aggressive in my feeding techniques which was causing Josiah to throw up a little more. The feeds were also taking a longer time. After a call back to the clinic I have a better idea of what I am doing in preparing the food – so hopefully things will get better.

I really do miss having Jessica around. She made the whole feeding process go so much smoother. Now if Josiah is slumping in the chair while eating I have no one to lift him up for me. (it is hard for me to do because my hands are all covered in goo!) Also, if he throws up I now have to stop the feed to clean it up – which I am just hoping is not reinforcing the throwing up behavior!

I think it is going to take a few days to readjust and get all the feeds completely back on track. Overall though he is still getting plenty of calories by mouth. It is also so much easier and less stressful than the feeding tube!!

Back Home in Kansas City

I’m exhausted…. sitting here on our couch in KC. It’s been a long intensive 3 weeks – but we are finally back home. I’m sorry I didn’t get round to a blog yesterday – it was kind of rushed and by the time I rolled into bed I really wanted sleep more than to blog…

So how has the feeding been going with Josiah – better and better – it seems now that each feed we are getting in to him over 500 cals – which is amazing – yesterday was a new record of 1850 cals in the day. Amazing as it’s all by mouth. We said goodbye to Ben and Jessica (Ben’s Assistant) finally last night – stocked up on all feeding supplies and Shari is now well equipped in the feeding protocol that we have to be very rigid about keeping for the next few months – after that we can look at different flavours and textures – which is apparently a very easy step up once Josiah has truly learnt how to swallow each bite properly. In the meantime we will stick with the high calorie food and get a little step up on the growth charts.

Today was going to be an interesting challenge – feeding Josiah while traveling all day – a challenge which we passed well – even if Josiah was tired and whiney the whole flight back from San Francisco – (Sorry to all passengers on the Mid West Flight). It really helps that we can get over 500 cals per meal in to him – then add in a few drink snacks and he was going great – Even though we only managed 2 main feeds we still managed to get around 1600 cals into him – and he drank quite a bit of this on the plane. One thing I love about feeding him by mouth is the fact that he does not spit up – we can feed him some milk by cup on the plane and I am confident he is not going to spit up – amazing.

So what’s my assessment of the three weeks

Week 1 – Josiah lost a bunch of weight (8oz) and feeding was hard – he cried alot and spit up alot during the meal – but the resistance gradually tapered off – the first weekend when we had to put in the tube again was not so pleasant – much spit up in many places all over the bay area. However the one saving grace is that he regained some of the lost ounces.

Week 2 – Still lost weight- but Tuesday/Wednesday a corner was turned – he ate a whole lot better – and from then on he has been going great – putting on weight. Eating more and more solids in one setting.

Week 3 – A week of parent training – It is not easy with NG babies – he still finds swallowing hard – and feeds are still 30-40 mins – but they are getting better. The weight gain which started last week took off – now Josiah is gaining an average of 3 oz per day – which is pretty amazing.

The primary purpose we went was to remove Josiah’s dependency on the NG tube – and we have achieved that very well

Weight Gain and Feeding

Well – we just came back from the last feed of the day a while ago – I think the 5oz weight gain yesterday kind of tuckered the little guy out – he always falls asleep in his car seat – but he didn’t wake up when we got back to the hotel – so no going to bed drink tonight.

Regardless – Shari did all the feeding today (barring his morning snack – which I did) – and she really did a marvelous job – the amount of solids he is getting each feed is increasing – 1.3oz, then 1.7oz – then tonight a whopping 3.1oz. She has got the knack – it will be only a matter of time for Shari to increase a little more in speed and amount – despite this he got the biggest amount volume wise and calorie wise of any day in the clinic yet – and as a result we are headed home on Saturday – back to Kansas City. Tomorrow we will learn how to make the high calorie stuff that we are feeding him – there are several Moms that have been to the clinic that have modified Ben’s formulas – one made each meal taste like what adults eat for Breakfast, Lunch and Dinner – a more recent client of Ben made the high calorie food and drink SUPER high calorie – making the 1.5 cal milk to 2.5 cal – each day her little boy gets over 2000 cals – and he is quite chubby.

We actually didn’t come to the clinic for weight gain – on the growth charts for a 10.5 month old he is between 25% to 50% of kids – for a 13 month old – which is his actual birth age he is between 5%-10% – so I guess getting some weight gain is an added bonus to getting off the tube and having him getting all of his nutrition orally which was our primary goal in coming to Los Altos. I think if we had done the night feed tonight we could have got his calorie intake to be more than 1800 cals – but I am not that concerned – I would prefer the little fella got his sleep. As must I.
